Job summary

Call on Clare is seeking a Community Support Worker to provide in-home personal care and support in Melbourne's Western Suburbs. You will help clients stay independent, assisting with daily living tasks and mobility.

The role emphasizes compassionate, client-centered care, coordinating with families and health professionals, and supporting dementia or palliative needs as required. Immediate start with flexible hours available to suit you.

Qualifications

• Certificate III/IV in Individual Support, Disability, or Ageing Support required or Diploma in Nursing with placement.
• Minimum of one year experience in similar or care industry.
• Current First Aid, CPR, and Manual Handling Certificate.
• 3 Doses of Covid Vaccine.
• National Police Check or willing to obtain one.
• Victorian Drivers License and own transport with Comprehensive Insurance.
• Working with Children check.
• NDIS Screening Check.
• Medication Administration Module.

Responsibilities

• Provide personal care including showering and dressing.
• Offer respite and social support in clients' homes.
• Support clients with exercises and activities and community participation.
• Assist with meal preparation and shopping.
• Transport clients to medical appointments and outings.
• Monitor clients' condition and report changes promptly.
• Provide dementia and palliative care as required.
