Timboon & District Healthcare Service in Warrnambool is seeking a compassionate Community Care Worker to support older people and those with disabilities to live independently at home. You will deliver personalised care, assist with daily tasks and promote wellbeing, while working with a team dedicated to dignity and choice.

The role values wellness, reablement and respectful, person-centred care, with opportunities across casual, part-time and full-time arrangements in a supportive rural

Qualifications

• Certificate III in Aged Care or Home and Community Care.
• Current Victorian Driver's Licence with access to a vehicle.
• National Police Check and Working with Children Check.
• Previous experience in community care is highly regarded.

Responsabilités

• Provide personalised care and practical assistance with daily living.
• Support independence and well-being through wellness and reablement.
• Monitor health and wellbeing and communicate concerns to the team.
• Work with clients, families and carers in a respectful, person-centred way.
