Qualifications

• Certificate III in Home and Community Services, Aged Care, Disability or equivalent.
• Current First Aid Certificate.
• Current Victorian Driver's Licence.
• Current National Police Check.
• Current Working with Children Check.

Responsibilities

• Deliver prescribed, goal-oriented services that support consumers to remain safe and independent
• Provide direct care in accordance with individualised care plans
• Assist consumers with daily living activities and lifestyle requirements using a person-centred approach
• Observe and monitor consumers' health and wellbeing and report changes as required
• Support consumers, carers, and significant others to build confidence in remaining at home
• Participate in regular care plan reviews in collaboration with consumers, carers, and the coordinator
• Maintain confidentiality, dignity, and respect at all times
• Ensure service delivery aligns with evidence-based practice, best practice standards, and effective risk management

Education

Certificate III in Home and Community Services, Aged Care, Disability or equivalent
