Job summary

Timboon & District Healthcare Service is seeking a compassionate Community Care Worker to support older people and individuals with disabilities to live independently at home. You will deliver personalised care, assist with daily tasks, and promote wellbeing and quality of life.

We value independence, dignity and person-centred care. A Victorian driver’s licence and vehicle are needed, with checks including National Police and Working with Children. A first aid certificate is advantageous.

Qualifications

• Certificate III in Aged Care or Home and Community Care as required.
• Current Victorian Driver’s Licence and access to a vehicle with comprehensive insurance.
• National Police Check and Working with Children Check required.
• First Aid Certificate would be highly regarded.

Responsibilities

• Provide personalised care and practical assistance with daily living.
• Support wellbeing and independence through wellness and reablement principles.
• Monitor health and wellbeing and raise concerns to ensure needed support.
• Build respectful relationships with clients, families and carers.
