Qualifications

• Certificate IV in Aged Care or relevant qualification in Nursing, Allied Health, Social Work, or related field.
• Demonstrated experience in case management or service coordination.
• National Police Check and Working With Children Check (or willingness to obtain).

Responsibilities

• Manage a caseload of participants and ensure they receive tailored support.
• Coordinate services and build relationships with participants and support networks.
• Ensure services are aligned with individual needs and personal goals.
