Job summary

Ozcare is hiring a part-time Registered Nurse to support in-home aged care across Gracemere and the Rockhampton region. You will assess client needs, develop care plans, administer medications, and provide holistic, evidence-based care aligned with professional standards.

Working with families and a multidisciplinary team, you will monitor progress, record outcomes, and contribute to quality improvement while maintaining compassionate, client-centred service.

Qualifications

• Proven high level verbal and written communication.
• Strong relationship management with stakeholders.
• Ability to work autonomously and in a multidisciplinary team.
• Comfortable using technology for rostering, assessments and documentation.

Responsibilities

• Initial assessment of client needs and care planning.
• Deliver holistic, client-centred care and coordinate with families and MDT.
• Administer and review medications, treatments and clinical interventions.
• Liaise with stakeholders and make referrals to support outcomes.
• Maintain accurate documentation and support quality improvement.
