Job summary

Centacare, operating under Archdiocese of Brisbane, seeks a Clinical Care Partner to deliver person-centred clinical care and coordinate services for older Australians in their homes across Caboolture. Travel to client homes is involved, with mileage reimbursed.

The role is full-time, Monday–Friday, 8:30am–4:30pm, offering salary packaging and professional development in a supportive, values-driven team environment.

Qualifications

• Bachelor of Nursing and current AHPRA registration as an RN.
• Valid driver's licence and access to a reliable, insured vehicle.
• Blue Card and NDIS Worker Screening Clearance required.

Responsibilities

• Complete clinical assessments, care planning and coordination.
• Monitor client health, escalate issues and support independence.
• Collaborate with clinicians, GPs and allied health professionals.
• Provide clinical guidance and supervise Care Partners and Support Workers.
• Ensure compliance with aged care standards and governance.
• Support investigations and manage care budgets with families.
