Calvary Home Care Adelaide is seeking a dedicated Case Manager to coordinate and deliver person-centred care for older Australians in their homes. Based in Adelaide, the role combines office work with regular community visits, managing assessments, care plans, and service coordination to promote independent living.

Ideal candidates will hold relevant aged care qualifications, a social work degree or equivalent, and experience in case management.

Formación

• Understanding of the new aged care act and support at home program.
• Full driver's licence.
• Experience in case management and coordination.

Responsabilidades

• Manage a caseload of clients, including assessments, care planning, and regular reviews to support independent living.
• Coordinate and monitor in-home services, working closely with clients, families, and service providers.
• Ensure compliance with Support at Home guidelines while maintaining accurate documentation and delivering person-centred care.

Educación

Certificate III in Aged Care

Certificate IV in Aged Care

Diploma of Community Services (Case Management)

Bachelor's degree in Social Work
