Job summary

Breakthru is seeking a Service Manager to lead day-to-day service delivery across designated homes or hubs, ensuring high-quality, person-centred support for people with disabilities. The role focuses on retaining customers by nurturing relationships and coordinating with participants to meet goals and community participation needs.

You will develop meaningful partnerships in the community, deliver seamless services, and lead a diverse team in line with the Disability Services Act and UN rights

Qualifications

Lead and manage day-to-day service delivery in homes/offices.
Improve quality of life for people with disabilities.
Develop partnerships with participants and stakeholders.
Interpret budgets and roster information to meet service goals.
Ensure compliance with disability service standards.

Responsibilities

Oversee efficient service delivery across designated homes or hubs.
Lead teams to deliver person-centred active supports.
Foster community partnerships to enhance engagement and outcomes.
