Momentum 360 Partners and Co. in Yarrawonga, VIC, is seeking entry-level and experienced Positive Behaviour Support Practitioners for a hybrid role with 0.7 FTE flexibility.

You will deliver individualised, evidence-based support to people with complex needs to improve quality of life and promote positive behaviour changes. Formación

• Bachelor’s degree in Education, Psychology, Nursing (RN1), Social Work, Speech Pathology or Occupational Therapy.
• Hold or can obtain an NDIS Practitioner ID, NDIS Workers Screening Check and Working with Children Check.
• Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with various stakeholders.
• Experience writing assessments and reports.

Responsabilidades

• Conduct comprehensive assessments to understand the individual's needs, strengths, and behaviour patterns.
• Develop and implement tailored positive behaviour support plans in collaboration with the individual, their family, and the support team.
• Provide training and mentoring to support workers to ensure consistent implementation of behaviour support strategies.
• Monitor and evaluate the effectiveness of behaviour support plans, making adjustments as needed.
• Collaborate with other professionals to ensure a holistic approach to support.
• Maintain detailed case notes and contribute to the organisation's continuous improvement processes.
