Job summary

Bayside City Council is recruiting a Care Partner to support its expanding Support at Home program. You will coordinate and monitor home care for older clients, balancing budgets and delivering high-quality, person-centred care.

Join an experienced team committed to flexible, hybrid work arrangements, and a strong focus on client outcomes. A tertiary health qualification and solid care coordination experience are essential.

Qualifications

• Bachelor-level qualification in health, allied health, social science or community services.
• Experience managing a caseload of older people with diverse and complex needs.
• Ability to develop and monitor individualised budgets, explain service costs and funding.
• Identify and respond to deterioration, unmet needs, risks, and escalate appropriately.
• Strong communication and relationship management, including sensitive conversations.
• Maintain accurate, timely care management records and use digital systems.

Responsibilities

• Coordinate care for Support at Home clients to enable safe, independent living at home.
• Join an aged care team and manage manageable caseloads to support reliable care.
• Monitor care quality, track actions and ensure effective use of funding.
• Collaborate with clients and carers to achieve positive outcomes and service satisfaction.
