You Matter is seeking a part-time Link Worker to support individuals with complex needs. Based in Lister, you will build therapeutic relationships, run activity-based interventions, and help residents resettle into their communities while supporting housing benefit processes.

• Build supportive and therapeutic relationships with service users through dialogue.
• Deliver activity-based interventions to promote wellbeing and independence.
• Assist in preparing service users to resettle into their community of choice.
• Ensure housing benefit applications are completed in a timely manner.
• Work with the support-worker team to meet daily needs of residents.
• Maintain accurate records and contribute to risk assessments and handovers.
• Engage service users in community activities and co-produce service plans.
