Qualifications

• Bachelor's degree preferred but not required.
• Previous experience working with individuals experiencing homelessness is strongly preferred.
• Valid driver’s license and reliable transportation.
• Willingness to travel throughout the assigned service area and periodically to WVCEH's office.

Responsibilities

• Provide individualized, community-based case management to approximately 15-20 households.
• Meet with participants at least weekly in their homes or other community settings.
• Assist participants with identifying and obtaining safe, permanent housing.
• Help participants obtain documentation and complete eligibility requirements for housing programs.
• Build relationships with landlords and property managers to identify opportunities.
• Provide ongoing housing stabilization and tenancy support, including landlord communication and budgeting.
• Connect participants with mainstream benefits and supportive services.
• Maintain HMIS client data in real time and within 48 hours of service delivery.
