Job summary

Melbourne City Mission is seeking two full-time Case Managers on a maximum term contract (up to 12 months) within the Homes First program to provide intensive goal-directed case management and outreach to people experiencing homelessness with complex needs.

You will work in a multidisciplinary team to support housing stability, develop tailored goals, and ensure a person-centred, trauma-informed approach while coordinating with care teams and external services to remove barriers.

Qualifications

• Manage a caseload of clients and develop advanced goal directed case planning and crisis interventions.
• Collaborate with care teams to assess needs, set goals, and develop strategies for independence.
• Assist people with complex needs to access ongoing accommodation options in line with their case plan.

Responsibilities

• Maintain up-to-date knowledge of MCM programs and external services for integrated support.
• Deliver services within MCM policies, procedures, and Victorian safety standards.
• Engage in assertive outreach to people experiencing homelessness to support housing stability.
