Job summary

Right at Home Gold Coast is seeking a compassionate Community Aged Care Physiotherapist to provide person-centred, evidence-based care in older Australians' homes and communities. You will assess, treat and educate clients, coordinate with a multidisciplinary team, and build lasting relationships

Qualifications

• Bachelor’s or Master’s degree in Physiotherapy.
• Current AHPRA registration.
• Strong clinical reasoning and assessment skills.
• Excellent communication and interpersonal skills.
• The ability to work independently within clients' homes.
• A commitment to person-centred and evidence-based care.
• An interest in aged care, rehabilitation, healthy ageing and maintaining independence.

Responsibilities

• Completing comprehensive physiotherapy and functional assessments.
• Developing individualised treatment and rehabilitation plans.
• Supporting mobility, strength, balance and functional independence.
• Falls risk assessment, prevention and management.
• Prescribing appropriate exercise and home-based rehabilitation programs.
• Assessing mobility and recommending appropriate aids or equipment.
• Educating clients, families and Care Partners about safe mobility and exercise.
• Maintaining accurate clinical documentation and care plans.
• Identifying changes in client health or function and escalating concerns appropriately.
