Job summary

EFNLHN seeks a committed Case Coordinator or Care Partner – Support at Home to join the Streaky Bay and Ceduna Community & Allied Health team. This full-time, ongoing role is open to ASO3 Case Coordinators and Enrolled Nurses, with appointment/classification based on qualifications and experience.

Key duties include coordinating, implementing, monitoring and reviewing individualised care plans, ensuring services align with the Aged Care Act 2024 and EFNLHN standards, and supporting safe,

Qualifications

• Enrolment or eligibility for Enrolled Nurse with NMBA, or experience in aged care coordination.
• Knowledge of Commonwealth home support programs and aged care legislative frameworks.
• Ability to work independently in a geographically dispersed rural region.
• Strong communication and a genuine commitment to person-centred, rights-based care.
• Experience coordinating across multidisciplinary teams.
• Commitment to delivering equitable health outcomes in remote communities.

Responsibilities

• Coordinate and deliver home support services under CHSP and Support at Home programs.
• Coordinate, implement, monitor and review individualised care plans reflecting participants' needs and goals.
• Ensure service delivery aligns with the Aged Care Act 2024, EFNLHN standards and program parameters.
• Provide clinical monitoring or comprehensive care management within your scope of practice (EN/ASO3) under supervision.
