Job summary

SA Health in the Mid-West region invites applications from Enrolled Nurses for the Support at Home program and CHSP. You will coordinate care plans, monitor services, and help participants access appropriate supports to maintain independent living at home.

The role supports EN stream with supervision from a Registered Nurse or ASO3 with broader care management responsibilities. Current NMBA registration or eligibility is required.

Qualifications

• Enrolment or eligibility for NMBA registration as an Enrolled Nurse (EN) in Australia.
• Experience in aged care coordination or community services case management (ASO3).
• Current practising certificate or eligible to obtain one.

Responsibilities

• Coordinating, implementing, monitoring and reviewing individualised care plans that reflect each participant's needs, preferences and goals.
• Ensuring service delivery aligns with the Aged Care Act 2024, Quality Standards, and program parameters.
• Supporting timely access to services and escalating concerns to promote participant safety and wellbeing.
• Providing clinical monitoring and care guidance within your scope of practice for EN (or care management for ASO3) under RN direction.

Job description

Working across the Mid-West region, you will support older people to remain living independently at home through the Support at Home program and Commonwealth Home Support Program (CHSP).

Key responsibilities

• Coordinating, implementing, monitoring and reviewing individualised care plans that reflect each participant's needs, preferences and goals.
• Ensuring service delivery aligns with the Aged Care Act 2024, Strengthened Aged Care Quality Standards, and EFNLHN program parameters.
• Supporting timely access to appropriate services and escalating concerns to promote participant safety and wellbeing.
• Contributing clinical monitoring and care guidance within your scope of practice (EN stream), or providing comprehensive care management and needs assessment (ASO3 stream), with EN practice under the direction and supervision of a Registered Nurse.

What you need

• Enrolment, or eligibility for enrolment, as a Nurse with the Nursing and Midwifery Board of Australia and a current or eligible practising certificate (EN applicants); or demonstrated experience in aged care coordination or community services case management (ASO3 applicants).
