Eyre and Far North Local Health Network's Country Health Connect – Home Support in Streaky Bay is seeking an experienced Home Care Coordinator for a part‑time role, 60 hours per fortnight, until 31 December 2026.

You will coordinate Commonwealth-funded aged care programs, conduct holistic assessments and develop person‑centred care plans for older clients living with independence and safety in their communities. This role requires collaboration across health, community and government partners.

Qualifications

• Experience in case management and coordinating care for people with complex needs.
• Strong knowledge of Commonwealth aged care programs (CHSP/Support at Home).
• Ability to conduct thorough assessments and translate findings into care plans.
• Excellent communication across multidisciplinary teams and communities.
• Experience working in rural or regional health settings is desirable.

SA Health's Eyre and Far North Local Health Network (EFNLHN) is looking for an experienced Home Care Coordinator to join Country Health Connect in Streaky Bay on a part-time basis, 60 hours per fortnight, until 31 December 2026.

What You'll Be Doing

Working within the Community and Allied Health team, you'll coordinate Commonwealth-funded aged care programs - including Support at Home and the Commonwealth Home Support Program - helping older people in Streaky Bay and surrounds remain living safely and independently. Your key responsibilities will include:

• Conducting holistic assessments and developing, implementing and reviewing person-centred care plans
• Coordinating referrals and services across health, community and government providers
• Advocating for clients while actively promoting independence, choice and self-advocacy
• Contributing to service improvement through data collection, evaluation and quality initiatives

What You'll Bring

• Demonstrated experience in case management and coordinating care for people with complex or multiple needs
• Sound knowledge of Commonwealth aged care programs, including Support at Home or CHSP frameworks
• Ability to conduct thorough assessments and translate findings into practical, client-led care plans
• Strong communication skills with experience working across multidisciplinary teams and community networks
• Confidence managing documentation, reporting and competing priorities in a regional or rural health setting
