Job summary

Lutheran Services in Gympie, QLD, seeks a Service Delivery Team Leader to guide Home Care Support Workers and ensure high-quality, person-centred care. Role involves leading a team, rostering and coordinating services across Gympie and Kingaroy sites.

You will work with the Service Manager to maintain compliant operations, build a positive team culture and deliver exceptional client outcomes. This office-based role requires travel between sites and a commitment to care excellence.

Qualifications

• Previous leadership experience (within home care, aged care or community services).
• Diploma in Aged Care (or equivalent).
• Strong organisational skills and ability to manage competing priorities.
• Experience in rostering, workforce coordination or service delivery.
• Excellent communication and relationship-building skills.
• A proactive, solutions-focused approach.
• National Police Check (required or to be obtained).

Responsibilities

• Lead and support your team, coaching and developing Support Workers.
• Deliver high-quality, person-centred care aligned with client plans.
• Oversee rostering, scheduling and workforce planning.
• Monitor service quality and address client feedback.
• Build trusted relationships with clients, families and staff.
