Empower Healthcare is seeking a Home Care Physiotherapist for Sutherland and St George. The role focuses on delivering comprehensive in-home physiotherapy within Support at Home and NDIS for participants in a flexible, part-time arrangement.

You will work with a multidisciplinary team to assess, design and implement tailored treatment plans, while educating families and maintaining thorough patient records. AHPRA registration and a strong committment to high-quality care are essential.

Qualifications

• Bachelor of Physiotherapy (or equivalent) is required.
• Registration with AHPRA is required.
• Police Check Clearance is required.
• Excellent verbal and written communication skills are necessary.
• Strong time-management skills and attention to detail are essential.

Responsibilities

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home via Support at Home/NDIS.
• Collaborate with other allied health professionals to provide a multidisciplinary care plan.
• Educate participants and families to optimise ability to live healthy lives within the community.
• Liaise with service providers to ensure effective communication.
• Work autonomously to prioritise tasks, maintain records and provide patient reports.
