Empower Healthcare seeks a Physiotherapist to deliver high-quality care to participants across Support at Home and NDIS in Eastern Sydney. You will assess, plan and deliver tailored physiotherapy services in clients' homes.

Join a supportive multidisciplinary team with registration with AHPRA, police check, and a passion for improving independence and quality of life for participants. The role involves home visits across Eastern Sydney and collaboration with other health professionals.

Qualifications

• Bachelor of Physiotherapy and registration are essential.
• Excellent verbal and written communication skills are required.
• Strong time-management skills and attention to detail are important.

Responsibilities

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan.
• Educate participants and their families to optimise their ability to live healthy lives within the community.
• Liaise with service providers to ensure effective communication.
• Work autonomously to prioritize and complete tasks, maintain records and provide patient reports.
