Overview

The Eyre and Far North Local Health Network (EFNLHN) is seeking a committed and person-centred professional to join the Elliston Community & Allied Health team as a Care Partner - Support at Home (Mid-West). This part-time, ongoing opportunity is open to both ASO3 Case Coordinators and Enrolled Nurses, with the appointment and classification determined by the skills, qualifications and experience of the strongest candidate.

What You'll Be Doing

Working across the Mid-West region, you will support older people to remain living independently at home through the Support at Home program and Commonwealth Home Support Program (CHSP). Your day-to-day work will include:

  • Coordinating, implementing, monitoring and reviewing individualised care plans that reflect each participant's needs, preferences and goals
  • Ensuring service delivery aligns with the Aged Care Act 2024, Strengthened Aged Care Quality Standards, and EFNLHN program parameters
  • Supporting timely access to appropriate services and escalating concerns to promote participant safety and wellbeing
  • Contributing clinical monitoring and care guidance within your scope of practice (EN stream), or providing comprehensive care management and needs assessment (ASO3 stream), with EN practice under the direction and supervision of a Registered Nurse
What You'll Bring
  • Enrolment, or eligibility for enrolment, as a Nurse with the Nursing and Midwifery Board of Australia and a current or eligible practising certificate (EN applicants); or demonstrated experience in aged care coordination or community services case management (ASO3 applicants)
  • Sound understanding of Commonwealth home support programs and aged care legislative frameworks
  • Ability to work independently across a geographically dispersed rural region
  • Strong communication skills and a genuine commitment to person-centred, rights-based care
  • Experience navigating complex care needs and coordinating across multidisciplinary teams
  • Alignment with EFNLHN's commitment to delivering equitable health outcomes for remote and rural communities
