Job summary

Eyre and Far North Local Health Network seeks a dedicated Case Coordinator or Care Partner - Support at Home for the Streaky Bay and Ceduna area. This full-time role offers opportunities in CHSP and the new Support at Home program, with appointment based on EN or ASO3 qualifications and experience.

You will coordinate care plans, ensure compliance with aged care standards, and collaborate with multidisciplinary teams to support remote communities in rural South Australia.

Qualifications

• Enrolment, or eligibility for enrolment, as a Nurse with the Nursing and Midwifery Board of Australia and a current practising certificate (EN applicants) or demonstrated experience in aged care coordination or community services case management (ASO3 applicants).

Responsibilities

• Coordinate and deliver home support services under CHSP and the new Support at Home program.
• Develop, implement, monitor and review individualised care plans reflecting needs and goals.
• Support timely access to services and escalate concerns to promote safety and wellbeing.
• Provide clinical monitoring or care management within EN or ASO3 scope under supervision.
