Job summary

The Eyre and Far North Local Health Network is seeking a part-time Care Partner - Support at Home (Mid-West) based in Elliston. The role suits either ASO3 Case Coordinator or Enrolled Nurse, with appointment and classification reflecting experience.

You will coordinate care plans for older people, ensure services align with national standards, and work across a rural region, applying person-centred, rights-based care under supervision where required.

Qualifications

• Enrolment or eligibility for enrolment as a Nurse with AHPRA.
• Experience in aged care coordination or community services case management.
• Understanding of Commonwealth home support programs and aged care legislation.
• Ability to work independently across a geographically dispersed rural region.
• Strong communication skills and person-centred, rights-based care.
• Experience coordinating across multidisciplinary teams.

Responsibilities

• Coordinating, implementing, monitoring and reviewing individualised care plans that reflect each participant's needs, preferences and goals.
• Ensuring service delivery aligns with the Aged Care Act 2024, Strengthened Aged Care Quality Standards, and EFNLHN program parameters.
• Supporting timely access to appropriate services and escalating concerns to promote participant safety and wellbeing.
• Contributing clinical monitoring and care guidance within your scope of practice (EN stream), or providing comprehensive care management and needs assessment (ASO3 stream), with EN practice under the direction and supervision of a Registered Nurse.
