Mercy Health in Hughesdale, Melbourne, seeks an Occupational Therapist to join a multidisciplinary Home Care team. You will deliver person-centred, evidence-based services to maintain or improve clients' independence and quality of life.

Responsibilities include home-based assessments, developing intervention plans, recommending assistive technology and home modifications, and collaborating with clients, families and carers to achieve meaningful outcomes.

Qualifications

• Bachelor or Master qualification in Occupational Therapy.
• Current AHPRA registration as an Occupational Therapist.
• Current Driver’s Licence.
• Experience in aged care or community health is desirable.

Responsabilités

• Conduct comprehensive occupational therapy assessments in clients' homes.
• Assess functional capacity, mobility, falls risk, and home safety.
• Develop, implement, and review goal-focused intervention plans.
• Recommend assistive technology, equipment, and home modifications.
• Deliver reablement and restorative care interventions.
• Support clients to maintain independence in daily activities.
• Collaborate with care partners, nurses, allied health professionals, families, and carers.
• Prepare clinical documentation, reports, and funding recommendations.
• Contribute to quality improvement, risk management, and compliance.
