Job summary

Hospital in Your Home in North Brisbane, Queensland, is seeking an experienced Occupational Therapist to join our frailty programs. This part-time/full-time role focuses on delivering high-quality home-based care to elderly and vulnerable clients, improving independence and wellbeing.

You will assess, plan and provide hands-on therapy, working within a multidisciplinary team to promote safety, ageing in place, and continual program improvement.

Qualifications

• Registered with the Australian Health Practitioner Regulation Agency (AHPRA)
• Minimum 1 year experience as an Occupational Therapist, preferably in home care or community settings
• Experience addressing the needs of elderly/disabled clients

Responsibilities

• Conduct comprehensive occupational therapy assessments and develop individualised treatment plans.
• Provide hands-on therapy to maintain or improve functional independence in daily living activities.
• Collaborate with the multidisciplinary care team for coordinated client care.
• Educate clients and families on safety and ageing in place strategies.
• Contribute to ongoing improvement of frailty programs through innovation and growth.
