Job summary

Southern Adelaide Local Health Network seeks a Physiotherapist to deliver goal-focused, restorative services to older adults in their homes in a part-time contract. Reporting to the GEM@Home Advanced Nurse Unit Manager, you will assess, plan, and implement physiotherapy care to restore independence and mobility in a community setting.

Qualifications

• Degree or qualification that entitles registration as a Physiotherapist with the Physiotherapy Board of Australia.
• Experience delivering restorative physiotherapy to older adults.
• Ability to assess and manage patients with varying complexity in a community/home setting.

Responsibilities

• Assess patients’ physical abilities, problems and goals using formal and informal assessments.
• Participate in interdisciplinary planning to enhance service quality.
• Provide information to patients/relatives about functioning and ongoing management.
• Provide consultative physiotherapy advice to team members and external agencies.
• Support development of an integrated, responsive team culture.
