Job summary

Bolton Clarke in Australia is seeking a Care Partner to lead care planning and service delivery under the Support at Home program, helping older people live safely and independently at home.

You will collaborate with GPs and allied health, monitor risks, maintain accurate notes, ensure compliance with quality standards, and support participants and their carers in making informed decisions.

Qualifications

• Minimum Cert IV qualification in aged care, or Diploma-level qualification in Nursing, Community Services, Aged Care, Case Management, or equivalent experience.
• Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services.
• Strong understanding of the Support at Home Program, Quality Standards, and aged care rights.
• Strong communication, empathy and cultural competence.

Responsibilities

• Collaborate with participants to develop individualised care plans, incorporating goals, service preferences and cultural needs.
• Liaise with general practitioners, specialists and allied health providers for integrated care.
• Review care notes and service data; identify and address emerging risks or changes in participant needs.
• Maintain accurate, timely care notes and ensure documentation supports outcomes under aged care standards.
• Ensure care-management claims comply with program rules, excluding admin tasks.
