Job summary

NHS Jobs is seeking a healthcare professional to conduct face-to-face, telephone, and online consultations, assess and manage acute and chronic conditions, and provide health promotion guidance in line with current guidelines.

The role involves prescribing medications, reviewing pathology results, making referrals, and performing home visits when clinically required, while maintaining accurate records and safeguarding vulnerable individuals within the care team.

Responsibilities

• Conduct face-to-face, telephone, and online consultations.
• Assess, diagnose, and manage acute and chronic medical conditions.
• Provide health promotion and preventative care advice.
• Prescribe medications in accordance with current clinical guidelines.
• Review and manage pathology results, correspondence, and clinical reports.
• Make referrals to secondary care and community services as appropriate.
• Undertake home visits when clinically required.
• Maintain accurate and contemporaneous clinical records using SystmOne.
• Safeguard vulnerable children and adults in line with local and national policies.
• Work collaboratively with Partners, nursing staff, pharmacists, care coordinators, social prescribers, mental health practitioners, and other members of the wider healthcare team.
• Participate in multidisciplinary meetings where appropriate.
• Support continuity of care and care planning for patients with long-term conditions and complex needs.
• Contribute to quality improvement initiatives and clinical audits.
• Participate in practice meetings and educational events.
• Support the achievement of QOF, Enhanced Services, and PCN objectives.
• Maintain professional development, appraisal, and revalidation requirements.
• Actively participate in significant event reviews and clinical governance activities.
• Process clinical correspondence and workflow tasks in a timely manner.
• Complete reports and medical records reviews as required.
• Ensure coding and documentation meet practice and contractual standards.

Multidisciplinary Working
