Job summary

South Western Sydney Local Health District invites an experienced Occupational Therapist to join the Liverpool Transitional Aged Care Program as a Level 3 clinician. The role is full-time permanent, based at Liverpool TACP, focusing on reablement for older adults transitioning from hospital care back to the community.

You will conduct home assessments, contribute to goal setting and client-centered treatment, support discharge planning, and participate in quality improvement initiatives while

Qualifications

Recognised Degree in Occupational Therapy with AHPRA registration.
Advanced clinical knowledge and experience in OT.
Experience in planning, implementing and evaluating local service delivery.
Strong organizational and time management skills.
Experience in clinical supervision or mentoring.

Responsibilities

Conduct home assessments and contribute to goal setting.
Implement client-centred treatment and support discharge planning.
Participate in quality improvement and evidence-based practice.
Support and supervise junior clinicians and students.
