Job summary

HiTalent Recruitment is seeking a Mental Health Support Worker to join a PHN-funded program in Taree. This role is full-time hours, with access to supervision and support, and will provide holistic, person-centred, integrated long-term support for individuals experiencing severe and complex mental health challenges.

You will work with a well-respected not-for-profit to walk alongside people experiencing mental health challenges, offering practical, emotional, social support and coordinated

Qualifications

• Previous experience providing support to clients presenting with mental health challenges.
• Excellent communication skills and the ability to collaborate effectively within a team.
• Hold a NSW Drivers Licence and understand recovery-oriented mental health practice.

Responsibilities

• Delivering person-centred, recovery-focused support that promotes independence and community participation.
• Completing psychosocial assessments and developing, implementing and reviewing individual recovery plans.
• Providing practical outreach, in-home and community-based support tailored to individual needs and goals.
• Conducting risk assessments, providing crisis support and contributing to safety planning.
• Connecting participants with health, housing, education, employment and other support services.
• Collaborating with families, carers and multidisciplinary teams to achieve positive recovery outcomes.
