Job summary

Department of Families, Seniors, Disability Services and Child Safety QLD – Wacol QLD is seeking a dedicated professional to deliver client-focused care within the Forensic Disability Service. You will develop, implement and monitor individual plans to improve life skills and independence, including leisure, meals and daily living activities, while maintaining dignity and respect.

You will assess risk with clinical teams, support goal achievement in IDPs, contribute to treatment and habilitation

Responsibilities

• Develop, implement and evaluate support plans to assist clients to improve life skills and independence.
• Monitor and observe at-risk clients and provide information to the clinical team.
• Support clients to achieve goals within their Individual Development Plan.
• Implement treatment and habilitation programs with clinical staff.
• Prepare reports and maintain documentation while ensuring confidentiality.
• Ensure safety of clients, staff and visitors and contribute to managing challenging behaviour.
