IRT Group is hiring a Home Care Partner to work with customers in NSW, ACT and Qld, helping them stay at home while meeting care needs.

You will manage cases, coordinate services, and review plans to support wellbeing. This role requires case management qualifications and 1–2 years in aged care, with hybrid work from office and home. Eligible regions include ACT, Far South Coast, Shoalhaven, Illawarra or South East Sydney/ South West Sydney.

Qualificações

• Must hold a Cert IV in Community Services or similar.
• Case management experience in aged care is essential.
• Strong communication and report-writing skills required.

Responsabilidades

• Receive and assess referrals, determine eligibility and arrange services.
• Contact new clients to discuss expectations, goals and available supports.
• Conduct comprehensive assessments to identify needs and create care plans.
• Coordinate services with customers and providers to meet goals.
• Regularly review plans via telephone and adjust as needed.
• Maintain accurate care plans and ensure funding compliance.
• Assist with data collection and accurate record-keeping.
