Job summary

My Guardian is seeking a compassionate Community Field Nurse in Sydney to deliver high-quality, person-centred care in clients’ homes. You’ll work autonomously while remaining connected to a supportive team and contribute to empowering individuals to live safely, independently and with dignity.

Required: current AHPRA registration; ability to work independently; excellent communication; good organisational skills; proficient with Google Docs and Microsoft Office.

• Current and valid AHPRA Registration.
• Ability to work independently.
• Excellent communication and listening skills.
• Compassionate, patient and caring nature.
• High level of organisational and time management skills.
• Competent using Google Docs and able to learn business systems quickly.
• Experience with care systems.
• Proficient in Office and Excel (or similar).
• Able to work in a fast-paced environment.
• Driver’s license with access to own vehicle.

Responsibilities

• Conduct comprehensive home assessments and design personalised care plans that truly meet each client’s needs.
• Lead, mentor and empower care teams, providing expert nursing guidance and ensuring seamless service delivery.
• Deliver hands-on clinical care, including wound management, medication administration and dementia assessments.
• Monitor client wellbeing, promptly identifying and reporting any changes in health or behaviour.
• Coordinate holistic, multidisciplinary care by liaising with hospitals, doctors and allied health professionals.
• Maintain accurate, compliant documentation aligned with policy, funding and regulatory standards.
• Build strong client relationships while fostering a high-performing, customer-focused culture.
• Support operational excellence through collaboration with My Aged Care, NDIS and internal administration teams.
