Job summary

My Guardian Group in New South Wales seeks a dedicated Community Field Nurse to deliver in-home care, lead care teams, and coordinate multidisciplinary services. You will conduct assessments, design patient care plans, and ensure seamless service delivery.

You will administer medications, monitor health, and liaise with hospitals and doctors while maintaining compliant documentation and strong client relationships. This role requires independence and empathy.

• Must have current AHPRA registration.

• Competent using Google Docs and able to learn business systems quickly.
• Must have experience with care systems.

About the Role

The Community Field Nurse is a front-line position acting as a representative of My Guardian providing the services offered by the company. Their duties are not limited to those listed as the position has a wide scope of duties at times dependent on the needs of the company.

This position suits someone who enjoys structure, works well under pressure, and thrives in a fast-paced care environment.

Key Responsibilities

• Conduct comprehensive home assessments and design personalised care plans that truly meet each client’s needs.
• Lead, mentor and empower care teams, providing expert nursing guidance and ensuring seamless service delivery.
• Deliver hands-on clinical care, including wound management, medication administration and dementia assessments.
• Monitor client wellbeing, promptly identifying and reporting any changes in health or behaviour.
• Coordinate holistic, multidisciplinary care by liaising with hospitals, doctors and allied health professionals.
• Maintain accurate, compliant documentation aligned with policy, funding and regulatory standards.
• Build strong client relationships while fostering a high-performing, customer-focused culture.
• Support operational excellence through collaboration with My Aged Care, NDIS and internal administration teams.
