BlueCare is seeking a Care Partner to deliver person-centred, high-quality care in the Maryborough area. You will oversee care planning and coordination of services, working closely with clinical teams to achieve optimal client outcomes from assessed needs.

This permanent full-time role involves driving to client homes, managing budgets, reviewing plans, and supporting clients to navigate the care system. A Certificate IV in Health or Community Service and a current Australian driver's licence

Qualifications

• Demonstrated understanding of client-centred, community-based care delivery.
• Excellent communication and clinical problem-solving skills.
• Ability to liaise with clinical teams and provide coordinated care.
• Experience with community funding streams and programs.

Responsibilities

• Develop and review care plans in collaboration with client and care team.
• Manage budgets and coordinate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs of clients.
• Continuously monitor and evaluate care plans and outcomes.
• Assist clients in navigating the care system and resources.
