Job summary

The Creative Care Company is seeking a compassionate Disability Support Worker or Nurse to assist a young participant in Narre Warren who requires intensive home-based support for both physical and intellectual disabilities, including seizure management.

You'll deliver person-centred care, support daily routines, community engagement, and collaborative care planning with families and healthcare professionals.

Qualifications

• Certificate IV in Disability or equivalent (essential).
• Experience with seizure management/epilepsy (mandatory).
• Experience with behaviours of concern and de-escalation.
• NDIS Worker Screening Check (mandatory).
• Working With Children Check (mandatory).
• Experience with intellectual disabilities and related conditions is highly desirable.

Are you a compassionate, experienced and qualified Support worker looking to make a real difference? Or perhaps you are an enrolled or registered nurse looking to make the move from hospital to a home setting?

We’re seeking a Disability Support Worker or Nurses to join our team and support a young participant living with a rare disorder that requires intesive support at home. The disorder requires care to support both a physcial and intellectual disability. Seizure management is included in the care requirements.

About the Role:

You’ll be supporting a complex participant in Narre Warren with a variety of daily tasks and meaningful activities, including:

Assisting with appointments, daily routines, and mealtimes

Engaging in fun and enriching activities at home and out in the community (swimming, music events, sports)

Implementing strategies and care instructions in relation to seizures and positive behaviour support.

Key Responsibilities:

Deliver person-centred support aligned with NDIS goals

Facilitate social and community engagement

Provide personal care in a respectful and dignified manner

Follow and implement individual support plans, clinical recommendations and therapy strategies

Maintain accurate progress notes and report incidents as required

Collaborate with families, team members, and healthcare professionals

Qualifications & Requirements:

Certificate IV in Disability or equivalent (essential)

Experience and/or Education in Seizure Management and/or Epilepsy (Mandatory)

Experience with behaviours of concern, de-escalation strategies, implementing behaviour support plans. (Essential)

NDIS Worker Screening Check (mandatory)

Working With Children Check (mandatory)

Experience with intellectual disabilities, ASD, ADHD, OCD, Lissencephaly, Prader-Willi Syndrome, Fragile X, or Angelman Syndrome is highly desirable

Strong communication skills and a genuine passion for supporting others

Valid drivers lisence, With appropriate car insurance (Essential)
