Job summary

South Western Sydney Local Health District is seeking an Enrolled Nurse to deliver high‑quality, patient‑centred care within a multidisciplinary team. The role covers stroke, cardiac and general medical patients across acute, rehabilitation and community settings.

You will support discharge planning, patient education and care coordination, with emphasis on evidence‑based practice and continuous service improvement. NSW Health vaccination requirements apply.

Qualifications

• Current Authority to Practice as an Enrolled Nurse (Division 2), General with the Australian Health Practitioners Regulation Agency (AHPRA).
• Demonstrated recent experience as a Registered Nurse in an environment appropriate to the relevant specialty.

Responsibilities

• Provide care to patients with stroke, cardiac and general medical conditions within a multidisciplinary team.
• Contribute to seamless care across acute, rehabilitation and community services through collaboration with hospitals and community providers.
• Support patient and family involvement in care planning, education and health promotion to optimise recovery.
