Job summary

Access Aged Care on the Sunshine Coast is seeking an endorsed Nurse Practitioner to provide advanced clinical care across Residential Aged Care Homes with autonomy and strong governance.

You will assess, diagnose, prescribe and care plan for residents, supported by dedicated nurses and in-house after-hours coverage, with no on-call or weekend requirements.

Qualifications

• Experience in aged care, gerontology, palliative care or chronic disease management.
• Advanced clinical experience in assessment, diagnosis, treatment, prescribing and health promotion.
• Strong clinical reasoning, communication and leadership capabilities.
• Experience in quality improvement and evidence-based practice.
• Sound knowledge of aged care legislation and regulatory requirements.
• Current driver’s licence, reliable vehicle and professional indemnity insurance.
• Comfortable using technology and clinical software systems.

Responsibilities

• Provide advanced assessment, diagnosis, prescribing and care planning for residents across Residential Aged Care Homes.
• Collaborate with on-site nurses, GPs, telehealth geriatricians and physicians.
• Operate with autonomy within a governance framework and minimise administrative burden.

1 to 4 days per week. Practise to your full scope as an endorsed Nurse Practitioner in aged care. Flexible weekday scheduling.

The Role

Working across Residential Aged Care Homes in the Sunshine Coast and surrounding areas, you will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents. You will work within a collaborative multidisciplinary model alongside on-site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.
