Job summary

Access Aged Care is seeking an endorsed Nurse Practitioner to practise with autonomy and clinical depth in Residential Aged Care Homes on the Sunshine Coast. You will deliver advanced assessment, diagnosis, prescribing and care planning for residents within a supported multidisciplinary team.

The role offers flexible weekday scheduling (1–4 days per week) and strong governance with operational support to protect your time for high-quality clinical care.

Qualifications

• Experience in aged care, gerontology, palliative care or chronic disease management.
• Advanced clinical experience in assessment, diagnosis, prescribing and health promotion.
• Strong clinical reasoning, communication and leadership capabilities.
• Experience in quality improvement and evidence‑based practice.
• Sound knowledge of aged care legislation and regulatory requirements.
• Current driver's licence, reliable vehicle and professional indemnity insurance.
• Comfortable using technology and clinical software systems.

Responsibilities

• Provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents.
• Collaborate with on-site nurses, GPs, telehealth geriatricians and Physicians in a multidisciplinary model.
• Work across Residential Aged Care Homes in the Sunshine Coast and surrounding areas.

The Role

Working across Residential Aged Care Homes in the Sunshine Coast and surrounding areas, you will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents. You will work within a collaborative multidisciplinary model alongside on‑site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high‑quality clinical care.
