Job summary

Blue Summit Support Services is seeking a dedicated Support Worker to provide person-centred assistance in participants' homes and their communities, guided by individual support plans and SCHADS classifications.

You will help with daily living tasks, transport, community access and wellbeing support, while maintaining confidentiality and professional boundaries. Experience is valued, but attitude and reliability are key.

Qualifications

• Reliable and dependable for participants and team.
• Respectful of people living in their life, home and community.
• Good communicators who clearly engage with participants, families and team.
• Engaged and proactive in supporting participants, not just supervising a shift.
• Observant to changes, risks and information requiring communication.
• Professional with understanding of boundaries, confidentiality and disability support responsibilities.
• Willing to learn and develop skills.
• Able to follow individual support plans, procedures and strategies.

Responsibilities

• Community access – supporting participants to attend activities, appointments, employment, education and access their community.
• Transport – providing safe transport to allied health, appointments, shopping and other activities.
• Daily living support – assisting with routines, meal preparation, household tasks, personal care and building independence.
• Engagement and prompting – encouraging participants to take part in activities, routines and work towards their goals.
• Behaviour and wellbeing support – following agreed support strategies and responding appropriately to changes in presentation, distress or behaviour.
• Reporting and communication – completing clear, timely documentation and communicating important information with the wider support team.
