Job summary

Side by Side Inclusive Services is recruiting a dedicated disability support worker to help participants develop independent living skills, including cooking, cleaning, budgeting and daily routines, both at home and in the community.

You will read and implement individual support plans to promote social participation, document progress clearly and communicate professionally with families and allied health professionals to support real outcomes and ongoing progress.

Qualifications

• Excellent written communication, documentation and reporting skills.
• Strong interpersonal skills and confidence to communicate with families and allied health professionals.

Responsibilities

• Support participants to develop independent living skills (cooking, cleaning, budgeting, routines).
• Provide meaningful community access to encourage independence and social participation.
• Implement individual support plans and goals.
• Encourage participant independence and build skills.
• Complete accurate progress notes and reporting.
• Communicate professionally with participants, families and health professionals.
• Identify opportunities to improve participant outcomes.
