Job summary

Complex Communities Foundation Ltd is seeking compassionate, reliable and experienced Support Workers to provide person-centred assistance to people with disability and complex support needs. You will help participants live safely, independently and with dignity, promoting choice, inclusion and community participation.

The role involves assisting with personal care, meals, transport where needed, and contributing to a supportive team environment while meeting required checks and documentation.

Qualifications

• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Willingness to complete required pre-employment checks.

Responsibilities

• Provide person-centred support in line with individual care plans.
• Assist with personal care, daily living activities and meal preparation.
• Support community access, appointments, shopping, and social participation.
• Maintain accurate progress notes and documentation.
• Identify changes in health, behaviour or needs and report them.
