CareChoice Pty is seeking passionate Disability Support Workers to provide high-quality, person-centred care to people with complex support needs in Victoria. This expression of interest covers roles across settings including home care, residential and community support.

Ideal candidates have a Certificate III/IV in Disability or related fields, current First Aid/ CPR, NDIS Worker Screening, WWCC, and a valid Victorian driver’s licence with a reliable insured vehicle.

Qualifications

• Experience supporting people with complex physical needs.
• Person-centred approach to care.
• Strong communication and problem-solving skills.
• Flexibility and reliability.
• Confidence working independently and as part of a team.
• Certificate III or IV in Disability, Individual Support, Aged Care, Community Services, or similar.
• Current First Aid and CPR.
• NDIS Worker Screening Check.
• Employee Working with Children Check.
• Current Victorian Driver’s Licence.
• Reliable, comprehensively insured vehicle.

Responsibilities

• Provide support across settings including Supported Independent Living, 24/7 complex supports, community access and transitional supports.
• Assist clients with daily living activities and personal care in a respectful, safe manner.
• Collaborate with families, carers and other professionals to coordinate holistic support.
• Document and monitor goals, progress and any changes in client needs.
