Job summary

CareChoice is seeking passionate Disability Support Workers to provide high-quality, person-centred care to people with complex support needs.

You may support across settings including Supported Independent Living, 24/7 complex supports, community access, and transitional capacity-building supports. We look for candidates with experience in complex physical needs, a patient, adaptable approach, and strong communication skills.

Qualifications

• We expect a person-centred approach to care.
• Experience supporting people with complex physical needs is preferred.
• NDIS Worker Screening Check and relevant licenses are required.

Responsibilities

• Provide high-quality, person-centred support in various settings (SIL, 24/7 complex supports, community access).
• Assist with daily living tasks, mobility, and independence goals.
• Collaborate with a multidisciplinary team to plan and implement supports.

Essential Requirements

• Certificate III or IV in Disability, Individual Support, Aged Care, Community Services, or similar
• Current First Aid and CPR
• NDIS Worker Screening Check
• Employee Working with Children Check
• Current Victorian Driver’s Licence
• Reliable, comprehensively insured vehicle
