Job summary

Swara is seeking an Experienced Female Disability & Mental Health Support Worker to provide personalised, client-centered care. You will work with participants to improve daily living, independence, and health outcomes, collaborating with families and a multidisciplinary team.

You will implement care plans, maintain records, and uphold professional standards while supporting diverse participants aged 14 to 60. A driver’s licence and ongoing development opportunities are valued.

• Strong empathy and commitment to supporting mental health and disability needs.

Responsibilities

• Provide one-on-one support for daily living activities.
• Implement personalised care plans to support mental health and independence.
• Maintain accurate client records and documentation.
• Adhere to organisational policies, procedures and legislation.
• Support participants in developing coping strategies and life skills.
• Offer intergenerational support to participants aged 14 to 60.
