Job summary

Kerima Care is seeking a Disability Support Worker to provide person-centred support for people with disability in participant homes and community settings. You will help with daily living activities, skills development and participation in home and community life, aligned with each participant's goals and plans.

Responsibilities include following support plans, reporting health and safety changes, assisting with medication when trained, and maintaining accurate records.

Qualifications

• Participant-centred approach promoting dignity, choice, independence and inclusion.
• Effective verbal and written communication and ability to document support objectively.
• Reliability, empathy, patience, sound judgement and professional boundaries.
• Ability to follow detailed support plans, safety instructions and workplace policies.

Responsibilities

• Provide respectful, individualised support with daily living activities as agreed and authorised.
• Support participants to develop and maintain daily-living skills and independence.
• Follow participant support plans, health and safety, and documentation procedures.
• Assist with community access, appointments, recreation and social participation.
• Observe changes in health, behaviour or wellbeing and escalate as required.
• Administer or assist with medication where permitted and trained.
• Follow infection control, PPE and hygiene procedures.
• Complete accurate progress notes and required documentation.
• Promptly report incidents, safeguarding concerns and hazards.
• Respect privacy, confidentiality and cultural identity.
• Work cooperatively with participants, families and health professionals.
