Job summary

MMT Care Pty Ltd. is seeking experienced Support Workers to empower participants with high physical support needs to live with dignity, independence and choice. You will provide personal care, mobility assistance, and ongoing support to promote wellbeing and participation in the community.

Ideal candidates bring hands-on experience with complex care, calm communication, and a commitment to consistent, respectful support for individuals and families across Victoria.

Qualifications

• NDIS Worker Screening Check
• HLTAID011 Provide First Aid
• HLTAID009 Provide CPR

Responsibilities

• Provide person-centred support that promotes dignity, choice, safety, and independence.
• Assist participants with daily living, personal care, mobility, manual handling, hoist transfers, and mealtime support.
• Support participants with complex care and behavioural support needs using calm, respectful, and consistent approaches.
• Encourage participants to build confidence, stay connected with their community, and work towards personal, social, and self-care goals.
• Complete accurate records and progress notes to support continuity of care and positive outcomes.
