Job summary

Responsibilities

• Foster person-centred practice placing employees and families at the centre of support planning
• Participate in daily check-ins to implement the daily support plan
• Deliver supports aligned with each individual’s plan and goals
• Support employees to maintain focus, productivity and skill development
• Engage with employees and families to ensure shared decision-making
• Communicate changes in needs to leadership for adjustments
• Maintain accurate records of progress and supports provided
