Anglicare NT in Alice Springs seeks an experienced Aged Care Coordinator to lead a team delivering quality home care and CHSP services. You will coordinate assessments, care planning and reviews, ensuring compliance with Aged Care Quality Standards and culturally safe service delivery for Aboriginal and Torres Strait Islander peoples and diverse communities.
Strong leadership and collaboration across stakeholders are essential.
Qualifications
- Diploma in Human Services, Community Services, Health, Disability, Aged Care or related field.
- Experience coordinating services for older people living at home.
- Strong knowledge of aged care legislation, reforms, quality standards and compliance requirements.
- Experience developing, implementing and reviewing care plans.
- Demonstrated leadership, supervision and mentoring experience.
Responsabilités
- Lead, supervise and support a team of Aged Care Support Workers.
- Assist with consumer assessments, care planning and care plan reviews.
- Ensure services are delivered in line with Aged Care Quality Standards and legislative requirements.
- Support culturally safe service delivery for Aboriginal and Torres Strait Islander peoples and people from diverse backgrounds.
- Monitor consumer budgets, documentation and reporting requirements.
- Participate in continuous improvement, quality reviews and audit processes.
